
Last evening, Partners Community Health (PCH) welcomed community leaders, partners, board members, Resident and Family Council representatives, and staff to its 2026 Year in Review event, an evening dedicated to celebrating connection, collaboration, innovation, and impact across the organization and the broader seniors’ care sector. The evening also celebrated the release of PCH’s 2025-2026 Annual Report, showcasing the organization’s accomplishments over the past year and audited financial highlights.
The evening opened with remarks from Tess Romain, President & CEO, PCH, who reflected on another year of meaningful progress and transformation. Tess highlighted continued growth across Wellbrook Place, the expanding reach of the Integrated Seniors’ Hub, and the importance of partnerships in strengthening integrated, person-centred care for aging adults in Mississauga and surrounding communities. The evening also marked the external launch of PCH’s 2026–2030 Strategic Plan, outlining the organization’s vision and priorities for the years ahead.
A special moment during the evening recognized Jeff Lozon, PCH’s inaugural Board Chair, as he concluded his term with the organization. Guest Michelle DiEmanuele, Ontario Secretary of the Cabinet, spoke about Jeff’s leadership, vision, and unwavering commitment to advancing innovative models of care during PCH’s formative years.
In recognition of his contributions, Jeff was presented with an award honouring his lasting impact on the organization and the community it serves.
In his remarks, Jeff expressed his thanks to PCH, the Board of Directors and to Wellbrook Place residents and families.

“We started as strangers, [and] we became friends and colleagues, working together to build something quite special,” says Lozon. “I will continue to watch the success of Partners and its success for many years, and I know the future will be bright, thank you for your kind words and thank you for the privilege of contributing to this organization.”
The evening’s keynote presentation was delivered by Sonia Sennik, CEO of Creative Destruction Lab. As part of her talk, she discussed how health care settings try to adopt emerging technologies and what CDL has learned about how innovation and adoption of tools can succeed.
“There are different layers of AI adoption, I think sometimes we can talk about it in this very omnipresent way – AI is everywhere, AI can do everything,” says Sennik. “It can’t actually and it needs to be curated and crafted for your specific use case. You can think of it as a tool that you need to harness and the earlier you can get involved with shaping how the tool is deployed, the more it will serve what you’re looking for it to serve.”
The event concluded with an opportunity for attendees to connect and continue conversations over refreshments in the welcoming environment of the Integrated Seniors’ Hub.
