Hosted at Wellbrook Place’s Seniors’ Hub, our community programs provide an opportunity for Seniors living in Mississauga to benefit from the services and expertise of our team at Partners Community Health.
The Spend the Day with Us Program (“the Program”) offers flexible, day-long support with personal care, healthcare, wellness activities, and social interactions, ultimately reducing Alternate Level of Care (ALC) rates, providing seniors on waitlists with immediate supports, enhancing community connections , and improving the quality of life for seniors.
The Program is host in the Integrated Seniors Hub (“the Hub”) at Wellbrook Place, offering a comprehensive suite of services to support seniors and their choices in Mississauga, to support healthy aging, aging in place and aging in community.
Designed for seniors needing varying levels of support with opportunities to participate in activities and receive services on select days. We tailor programs to our client's individual needs, ensuring personalized care and engagement.
By integrating personal care, healthcare, recreational activities, and nutritional support we provide comprehensive physical, mental, emotional, and social care.
Personalized care plans based on unique needs and preferences with regular assessments and adjustments to ensure appropriate support.
Organized activities and services provide structure and security with the option for flexible participation based on daily condition and preferences.
Fostering a vibrant community for socializing and making new friends and encouraging social interaction to combat loneliness and promote well-being.

In collaboration with Mississauga Health (Mississauga Ontario Health Team), Caredove, CareTeam, Central Registry and Trillium Health Partners, this navigation portal will help older adults continue to live well at home by connecting them to health care providers in Mississauga.
To use this navigation portal, adults who are 65+ or their caregivers living in Mississauga can complete a 5-minute questionnaire which will direct them to book an appointment with a Care Navigator or a community support provider. From there, individuals will be connected to one or more community support services that can help provide wraparound care for adults or their caregivers.
A Dementia Café is a safe, supportive space where individuals living with dementia and their caregivers can come together in a relaxed, café-style setting. Here, participants can enjoy social interaction, share experiences, and engage in structured activities designed to stimulate the mind and uplift the spirit. With offerings like music, art, games, and talks by health professionals, the Dementia Café provides connection, understanding, and support for everyone affected by dementia. The dementia café also serves as a networking environment for caregivers who will be connected to services and resources to support their loved ones needs.
With dementia on the rise across Canada, spaces like our Dementia Café have become essential. According to the Alzheimer Society of Canada 2024, approximately 733,040 Canadians are living with dementia, with projections reaching nearly 1 million by 2030. In Ontario alone, dementia prevalence is expected to rise by 202% between 2020 and 2050.
This growing need inspired us to create our Dementia Café—a safe, understanding environment where individuals affected by dementia and their caregivers can connect, engage in meaningful activities, and experience a sense of belonging. Our café is designed to support healthy aging, promote aging in place, and foster aging in the community, ensuring participants can thrive in a supportive, dementia-friendly setting.
Through programs that enhance social, cognitive, physical, and nutritional well-being, our Dementia Café aims to improve quality of life, slow cognitive decline, and reduce caregiver stress. By fostering independence and community engagement, we strive to create a compassionate model for community-based dementia care, supporting dignified, enriched aging in place and advancing Wellbrook Place’s mission of holistic, integrated senior care in Mississauga.
