Partners Community Health (PCH), Trillium Health Partners (THP) and the Government of Ontario are revolutionizing long-term care (LTC) in the City of Mississauga with the opening of Wellbrook Place, two new state-of-the-art LTC homes, part of a larger Seniors’ Campus of Care, focused on providing specialized services to residents in the homes and the community. Wellbrook Place brings 632 much-needed new LTC beds to the city and is part of PCH’s larger plan that introduces innovative and inclusive programs and services, and new models of care delivery to families and communities.
