#PeopleofPCH is a series that highlights the diverse stories of staff, residents, volunteers, and families across Partners Community Health (PCH).
As part of our Innovation theme this month, we are proud to spotlight the Peritoneal Dialysis (PD) team at Wellbrook Place—nurses who bring a highly specialized and unique service directly to residents’ bedsides.
Wellbrook Place is one of the few long-term care homes in Mississauga able to provide bedside peritoneal dialysis. This service allows residents with kidney failure to receive treatment safely and comfortably in their own rooms—rather than traveling to a dialysis clinic multiple times a week or relying on family members to perform the procedure.
“We’re a very diverse team. We work on different neighbourhoods across Wellbrook Place East, but unite together to treat our PD patients,” says Bella.
The PD team—Edna, Basilia, Ana, Bella, and Marisol, along with Nam and Joyce, who provide primary PD care at night—have all completed specialized training to safely deliver this complex treatment. They collaborate closely to ensure the process is smooth, safe, and comfortable for residents.
“It’s good because it alleviates the pressure on families,” shares Basilia. “In the past, family members might have had to perform dialysis themselves or arrange for their loved one to travel to a clinic. Now, residents can stay at home, in familiar surroundings, while receiving the care they need.”
Beyond convenience, the program reflects PCH’s value of creativity and commitment to innovation. By reimagining how dialysis can be delivered, the PD team has eliminated scheduling challenges, reduced stress for families, and empowered residents to live more comfortably.
When asked what makes them most proud, the team agrees it’s their ability to collaborate across units and support residents with ease. “It’s a lifeline,” Ana reflects. “We’re not only delivering medical care—we’re helping residents and families feel more at home.”
The Peritoneal Dialysis program at Wellbrook Place is a powerful example of how teamwork and innovation come together to improve lives—right at the bedside.
