On November 7, 2024, Partners Community Health (PCH) celebrated a truly remarkable milestone – our one-year anniversary since opening the doors of Wellbrook Place, Ontario’s largest long-term care home. This past year has been one of resilience, growth, and profound impact. As we reflect on everything we’ve accomplished together, we want to share our immense pride and gratitude with each of the staff, residents, families and partners, the heart and soul of this incredible home.
Our vision was ambitious – to create a space that redefines long-term care, prioritizes dignity and compassion, and feels like home for our residents and their families. Through the unwavering dedication of our staff, residents, families and partners, this vision has become a reality. Every day, they bring our values of compassion, excellence, inclusivity and creativity to life, striving to ensure that each resident feels cared for, respected, and valued. This success is not just in our size but in the quality of care we provide, the joy we create, and the community we’ve built together.
Let’s take a moment to celebrate just a few of the many accomplishments that have made this year unforgettable:
- Welcomed over 750 residents since opening, providing them with comfort, safety, and community.
- Hired and onboarded over 700 staff where every role contributes to a collaborative environment and makes a difference.
- Grew resident programs and activities to activate residents on a daily basis, including partnering with local faith organizations to ensure services are available for residents based on their religious needs, such as Roman Catholic Mass and Jumma Namaz for our Catholic and Muslim residents.
- Built partnerships that support our growth and advancement, including collaborating with numerous organizations that have helped inform how we can best support residents at Wellbrook Place, as well as the needs in the community.
- Launched our Volunteer Program, which now has 39 active volunteers who play a critical role at Wellbrook Place including supporting resident and client programming, reception and welcoming visitors, meal and snack assistance, administrative and clerical support and sharing their various gifts and talents with residents, families and staff.
- Launched our Quality Improvement Plan, which is designed to support our goal of delivering the highest standard of care at Wellbrook Place, focusing on resident safety, personalized care, and care provider engagement.
Resident and Family Satisfaction Survey
To build a strong quality program, it is paramount that we listen to the feedback of our residents and families. As such, we are excited to announce that we also launched Wellbrook Place’s Resident and Family Satisfaction Survey on November 7. This survey is an important way for us to hear directly from residents and families about their experiences, insights, and areas of improvement. The feedback received will guide us in enhancing our programs, services, and overall environment at Wellbrook Place.
All of this impressive work was made possible thanks to the dedication and hard work of our clinical teams, leadership, corporate services, resident and family councils and seniors hub team, who have helped make Wellbrook Place a warm home for residents and families and staff.
We are grateful to each and every one of them for their dedication and hard work over this past year. Their commitment to excellence and compassion has made Wellbrook Place a place our residents are proud to call home. Together, we’ve built a foundation of care, respect, and community, and their efforts have been the heart of this success.
To our residents and families, thank you for your trust, support, and partnership over this past year. You inspire us and motivate us to reach new heights every day.
And to every member of our team – from the frontline care providers to the dedicated support staff in every department, and management teams – thank you for making Wellbrook Place what it is today.
