Partners Community Health (PCH) has launched its 2026–2030 Strategic Plan, setting a clear direction for the organization as it continues reimagining the future of integrated seniors and aging care.
The new plan outlines four strategic priorities that will guide PCH’s work over the next four years, focusing on:
- Elevating Our People
- Delivering High-Quality Care
- Driving Connected Care for Seniors and Aging Adults in the Community
- Advancing Academics and Research in Aging Care
The strategy was informed through extensive engagement with residents, clients, families, staff, physicians, community partners, and the Board of Directors.
“This strategic plan reflects the voices of our community and the people who make our work possible every day,” says Tess Romain, President and CEO of Partners Community Health. “As the needs of seniors and aging adults continue to evolve, this plan ensures we are prepared to respond with compassion, innovation, and excellence in care.”
Over the next four years, PCH will focus on four strategic priorities:
- Elevate Our People for the Future: Foster a culture of continuous learning, professional growth, and team well-being to ensure high-quality care and future readiness.
- Deliver High-Quality Care: Enhance care delivery, improve resident outcomes, and streamline operations across our organization.
- Drive Connected Care for Seniors and Aging Adults in the Community: Support seniors and aging adults by establishing an interconnected range of services, supports, and care that enable aging in place with dignity and choice.
- Advance Academics and Research in Aging Care: Position our organization at the forefront of aging care by leveraging innovative digital tools and technologies, promoting academic collaboration and research and implementing innovative care models.
These priorities will be enabled by digital transformation, sustainable growth, and partnerships.
Together, this Strategic Plan will guide how PCH continues to strengthen services, support its teams, and improve care for seniors and aging adults across the community.
